doc section 3.4.1: correct first paragraph

This is a reversion or a duplicate of a bug which was closed as rejected in
2006:

Whatever the situation then, the reason it was rejected is not currently valid:
section 3.4.1 does not at present state the problem with the h suffix. It
mentions a similar problem with the $ prefix, so there's no reason it
shouldn't. It gives a a valid example for h suffix usage (0c8h), but that isn't
any more than a small, easily overlooked hint at the problem.

I've attached a diff which adds this information to the first paragraph of
section 3.4.1, merging it with the $ prefix caveat. I hope I've done a good job
of it.
